MedGemma Impact Challenge: Offline Triage App for Sailors

The MedGemma Impact Challenge ran on Kaggle from January 13 to February 24, 2026, with more than 850 teams participating.

As part of the MedGemma Impact Challenge, Rick created the SailingMedAdvisor application. There were comparable submissions geared to providing medical decision-making information to rural areas or low resource clinics.

While none of the project comparables were winning entries, the volume of interest in this area demonstrates an interest in using AI to meet the need for this information.

About SailingMedAdvisor

SailingMedAdvisor is a fully-offline, AI-assisted medical support system designed specifically for offshore sailing vessels operating without reliable internet access. It reflects the practical constraints of life at sea: a fixed crew, limited medical supplies, constrained hardware, and high-stress decision-making during medical events.

Here is a video that describes SailingMedAdvisor, and how it can be used in real-world situations.

SailingMedAdvisor Features

FeatureSailingMedAdvisor
Purpose-built environmentDesigned specifically for medical problems aboard offshore sailing vessels rather than for remote healthcare generally.
Boat informationIncludes information about the specific vessel and its onboard capabilities and constraints.
Passenger and crew listMaintains information about the actual people aboard rather than treating each case as an unknown patient.
Individual medical contextCan use relevant medical information associated with individual passengers and crew when assessing a problem.
Pharmaceutical inventoryKnows which medications are actually onboard, allowing recommendations to reflect what is physically available.
Medical supplies onboardCan take the boat’s available medical equipment and supplies into account rather than assuming access to normal clinical resources.
Offshore constraintsDesigned for situations where professional medical care may be hours or days away and immediate referral to a hospital may not be practical.
Fully offline operationDesigned to function without internet access, using local hardware aboard the vessel.
